*Local clients only (Belgrade/Bozeman). $15 surcharge for travel to Livingston/Three Forks/Ennis community.

After you book an appointment, you will receive an email with further instructions. You will be directed to read over some brief policies & procedures and asked to sign a ‘Client Consent for Care’ form.

Depending on your requests and concerns, a full client intake form (mother & baby health history) may be needed in order to provide the best possible care outcome.
