Consent for Care
MilkMaid Babies provides all clients with a Consent for Care. This form will be provided to the client when scheduling the visit, and must be signed before services are rendered.
Consent is verbally requested before touching client or client’s minor child. Client may withdraw consent to be touched even if consent was previously given verbally or in writing.
HIPAA Compliance
When it comes to your health information, you have certain rights. This form will be provided to you upon booking and explains your rights and some of my responsibilities as your lactation consultant.
Cancellation Policy
I understand that things happen! If you need to cancel your appointment due to unforseen circumstances please notify me as soon as possible or at least 2 hours in advance. We can discuss rescheduling options at that time.
To be respectful of your baby’s health, client visits will be canceled in the event of provider illness or personal emergency. Rescheduling is not guaranteed, but all efforts will be made to rebook the appointment. In the event of cancellation for illness or personal emergency, clients will be notified by their preferred method as provided during intake.
Payment Policies
Self-pay clients: You agree to submit payment at the time of the visit via cash, check, credit card (preferred methods) or PayPal, Venmo, Zelle (on a case by case basis). At your request, I will provide you with a superbill suitable for you to submit to your insurance. The superbill (which will also serve as a payment receipt) will be coded appropriately to the level of service provided during the visit.
